Referrals
For F&CS Protective Service Workers:
When a youth is 14 years old, send a copy of the youth’s Transitional Independent Living Plan (TILP) to: Arlene Hylton, ILSP Coordinator, 0J60, 225 Valencia Street. This includes youth who are residing in out of home care, whether they are living in San Francisco or other counties. This is not applicable to Family Maintenance (FM) cases. ILSP needs to know of changes in address, social worker, phone number, etc., as well as any significant changes in the case plan. For San Francisco Probation Officers:
The San Francisco Probation Officer needs to send a copy of the youth's Transitional Independent Living Plan (TILP) if the minor is residing in San Francisco to: Arlene Hylton, ILSP Coordinator, OJ60, 225 Valencia St. Fax: 415-934-4228
